I would like to apply for the coverage with the company according to general condition of the policy, and to certify that above health condition declaration is all correct and true, and to verify that I have a perfect health condition and have no disability, and to agree to use this application as a basis of the insurance contract between me and the company. Should there be any false statement or any truth being concealed, I agree to let the company cancel this insurance policy. I, besides this, assign Thai Health Insurance Public Company Limited to request for any kind of detail or information, regarding my personal healthy treatment or health condition records from any physician, hospital, clinic or any other organication which has or knows my healthy condition records, including HIV test result.

ผู้ขอเอาประกันภัยต้องตอบคำถามในใบคำขอเอาประกันภัยตามความเป็นจริงทุกข้อ การปปกปิด หรือแถลงความเท็จใดๆ อาจเป็นเหตุให้บริษัทผู้รับประกันภัยปฏิเสธไม่จ่ายเงินค่าสินไหมทอแทนตามสัญญาประกันภัย ตามประมวลกฏหมายแพ่งและพาณิชย์ มาตรา 865 หากมีข้อสงสัยใดๆ เกี่ยวกับกรมธรรม์ประกันภัยนี้ กรุณาติดต่อขอคำแนะนำได้จาก บริษัท ไทยประกันสุขภาพ จำกัด (มหาชน) หรือจากสำนักงานคณะกรรมการกำกับและส่งเสริมการประกอบธุรกิจประกันภัย ที่โทรศัพท์ 0 2515-3995 หรือสายด่วน 1186 /

Any concealment or false statement may result in the insurance company refusing to pay claims, according to The Civil and Commercial Code, Section 865.
