Applying for Individual Health and Personal Accident Insurance
SIMPLY HEALTHY, MAXI HEALTHY, WEALTHY HEALTHY, CASHY HEALTHY


1. Completely fill both sides of the Individual Health and Personal Accident Insurance Application Form and sign the applicant’s name in the application, in case the applicant is less than 20 years of age the parent must cosign in the application.
2. Submit the application and supported documents to the company and make premium payment by the coverage plan that the applicant purchases.
  • In case the company agrees to insure without conditions, the company will issue the insurance policy and health insurance card and send to the applicant within 7 days.
• In case the company needs to have more information or to impose any condition in the policy, the company will inform the applicant on case by case basis.
• In case the company refuses to insure, the company will inform the applicant and refund premium in full.

Applying for Group Health and Personal Accident Insurance - EMPLOYEE HEALTHY

1. Completely fill both sides of the Group Health and Personal Accident Insurance Application Form for the Policy Holder (Employer’s Application) and sign the authorized person’s name in the application.
2. Completely fill both sides of the Group Health and Personal Accident Insurance Application Form for the Covered Person (Employee’s Application) and sign the applicant’s name in the application.
3. Submit the Group Health and Personal Accident Insurance Application Form for the Policy Holder (Employer’s Application) and the Group Health and Personal Accident Insurance Application Form for the Covered Person (Employee’s Application) of every employee and supported documents to the company.
4. The company will calculate all premium and issue invoice to the policy holder to make premium payment.
5. The company will issue the policy and health insurance card and send to the policy holder within 7 days
Supported Documents for Applying for Group Health and Personal Accident Insurance - EMPLOYEE HEALTHY
1. Group Health and Personal Accident Insurance Application Form for the Policy Holder (Employer’s Application)
2. Company Registration Certificate or VAT Registration Certificate (๏ฟฝ.๏ฟฝ. 20)
3. Group Health and Personal Accident Insurance Application Form for the Covered Person (Employee’s Application)
4. – Copy of ID Card of the applicant, or copy of Passport in case of foreigner
    – Copy of Marriage Certificate (In case of cover spouse dependant)
    – Copy of Birth Certificate of children (In case of cover children dependant)
5. In case that you have previously had Group Health Insurance with other insurance company, please submit copy of the policy and covered persons list in order for the company to provide continuous coverage.
6. In case that you need to have claim payment transfer to the covered person’s bank account, please also notify covered person’s bank account number to the company.
Applying for Personal Accident and Health Insurance for Traveling - JOURNEY HEALTHY

1. Completely fill the Personal Accident and Health Insurance Application Form and sign the applicant’s name in the application, in case the applicant is less than 20 years of age the parents must cosign in the application.
2. Submit the application and supported documents to the company and make premium payment by the coverage plan that the applicant purchases.
3. The company will issue the insurance policy and send to the applicant within 1 day.

Health Insurance Card Usage Procedure
In case you have medical treatment in the network hospital


IPD Case
At the Admission Stage
1. Present health insurance card together with ID Card or Passport to the network hospital’s personnel

2. Sign in the medical claims form for the hospital

3. Waiting for coverage confirmation
from the company through fax claim service

4. Admission at the hospital
At the Discharging Stage

1. The hospital concludes medical treatment expenses
and informs the company for your discharging

2. Waiting for coverage amount confirmation
from the company through fax claim service

3. Pay for the excess claims amount to the hospital (if any)

4. Discharge from the hospital
Remark: The company reserves the right to approve medical treatment coverage for IPD in the network hospital through fax claim service on case by case basis
OPD Case
 
1. Present health insurance card together with ID Card or Passport to the network hospital’s personnel

2. Sign in the medical claims form for the hospital

3. Receive OPD Treatment

4. Pay for the excess claims amount to the hospital (if any)

5. Discharge from the hospital
 
Remark: In case the company has already approved medical treatment coverage for you but find out later that such coverage is not covered under the policy, i.e. the sickness causes from pre-existing condition, the company reserves the right to recollect such medical treatment expenses back from you, by issuing an invoice to you later.
 
Waiting Period for Individual Health and Personal Accident Insurance
SIMPLY HEALTHY, MAXI HEALTHY, WEALTHY HEALTHY, CASHY HEALTHY


• The company will not pay the benefits for any sickness during the first 30 days from the first policy commencement date, except injury from accident will be covered immediately from commencement date.
• The company will not pay any benefits during the first 120 days from the first policy commencement date for the following diseases:
  • Benign or malignant tumor or cancer or cystic mass
• Hemorrhoids
• Hernias
• Pterygium, Pinguecula, Cataract
• Tonsillectomy or adenoidectomy
• Stones
• Varicose Veins
• Endometriosis
Claims Reimbursement Procedure
In case you do not have medical treatment in the network hospital


There might be some reasons preventing you to have medical treatment in the network hospital i.e.
  - Your preferred hospital is not in the hospital network
- You choose to have medical treatment in government hospital which is not in the hospital network
- You are traveling or staying abroad
- Your other reason

You are still covered by the policy, even you, by any reason, do not have medical treatment in the network hospital, but you have to pay for medical treatment expenses by yourself first and then reimburse from the company later. In this case, the documents for reimbursement include:
  1. Claims Form for reimbursement purpose

2. Original Copy of Attending Physician’s Report which indicates diagnosis and treatment method
3. Original Receipt which indicates details of treatment expenses

You can either send above documents to us by hand or by registered mail to Claims Division, Thai Health Insurance Company Limited, 121/89 RS Tower Building 31st Fl., Ratchadaphisek Rd., Dindaeng, Bangkok 10400

You should send above documents within 30 days after discharging from hospital. In case you have any proper reason preventing you to send them within the specified period, you can send them after that. However, the claims reimbursement cannot be made after 2 years from the admission date.

The company shall pay your claims within 7 working days from the date we receive your complete claims documents, unless we needs to investigate any further information. In this case, we shall pay your claims within 90 days from the date we receive your complete claims documents.

You can request your payment method either by account payee check, or pay-in into your bank account. In case you need us to pay-in into your bank account, please notify us your account number prior.

Worldwide Emergency Assistant Coverage (Provided by International SOS)
In case of purchasing of worldwide emergency assistant extended coverage MAXI HEALTHY, WEALTHY HEALTHY, EMPLOYEE HEALTHY, JOURNEY HEALTHY
Evacuation and Repatriation
• In case the insured has injury or sickness in the rural area, the insured can contact local physician for first aid treatment, after that International SOS will provide evacuation means and coordinate with the local doctor for further treatment. In this case, the insured must ask for attending physician’s report and original receipt, besides that, the company and International SOS will further coordinate for the insured.
• International SOS will be responsible for arranging transportation to evacuate the insured for emergency treatment at the nearest hospital that has quality treatment facility, and will be responsible for all evacuation expenses but not exceed the coverage benefit stated in the policy.
• International SOS will be responsible to repatriate the insured for further treatment in Thailand, and will be responsible for all repatriation expenses but not exceed the coverage benefit stated in the policy.
Repatriation of Mortal Remain

In case the insured dies in foreign country, International SOS will be responsible to repatriate the body remain of the insured to the country of origin and will be responsible for all repatriation expenses. In case of there is need to cremate the insured body at the place that the insured dies, International SOS will be responsible for all expenses including cremation expense but there must be prior approval from International SOS.
